Who we are

Larrivée Medical Cosmetic Clinique, also known as Clinique Larrivée, is a medical aesthetics clinic based in Timmins, Ontario. Treatments are provided by Registered Practical Nurses, Cynthia Larrivée, RPN, and Rachel Otis, RPN, under the medical directive of Susan Jomha, NP, our Medical Director.

As a health care provider in Ontario, we follow the Personal Health Information Protection Act, 2004 (PHIPA). This page explains, in plain language, what information we collect, why we need it, and how we keep it safe.

What we collect

We only collect what we need to care for you safely:

• Contact details such as your name, date of birth, phone number, email and address.
• Health information such as your medical history, medications, allergies, consent forms and treatment notes, including the products and lot numbers used.
• Photos taken before and after treatment to plan your care and track your results.
• Booking and payment records such as appointments, invoices and receipts.

Your quiz and LMCC AI Analysis answers, including health screening answers such as pregnancy, medications or conditions, are health information. We use them only to follow up with you and never for advertising.

Why we collect it

• To check that a treatment is safe and suitable for you.
• To provide your treatment and keep an accurate health record, as nurses in Ontario are required to do.
• To book appointments, send reminders and follow up on your results.
• To process payments and keep our business records.
• To meet our legal and professional obligations.

Your photos are part of your private health record. We will never use them on social media, our website or in any advertising unless you give us separate written permission, and you can take that permission back at any time.

Who sees your information

Your information is seen only by the people involved in your care at the clinic, and by our medical director when needed for your care. We never sell, rent or trade your personal information.

We only share it outside the clinic:

• When you ask us to or give us permission.
• With another health care provider involved in your care, where the law allows it.
• When we are required by law, for example by a court order or our regulatory college.

Booking and other services we use

We use a few trusted services to run the clinic. Each one is expected to protect your information and use it only to provide its service to us.

• Jane, a Canadian practice management platform built for health clinics, for online booking, intake forms, reminders and records.
• Beautifi, if you choose to apply for a payment plan. Your application is handled by Beautifi under its own privacy policy.

We also use Mailchimp to send our email and text newsletter, and Jotform to deliver your quiz and LMCC AI Analysis results to our clinic email. Our website chat and LMCC AI Analysis use Cloudflare Workers AI to answer questions and, only if you agree, to analyse the photos you add. Photos are not kept by the AI. If you choose to send your LMCC AI Analysis to the clinique, your answers, report and photos are stored securely and deleted automatically after 90 days. Some of these services store information outside Canada, including in the United States, where local laws apply to it.

Our website and messages

When you visit our website, basic technical details such as your browser type and the pages you view may be collected through cookies and analytics tools. This helps the site work properly and shows us how it is used. It does not tell us who you are. You can block or delete cookies in your browser settings.

Our website uses Google Analytics to count visits and see which pages are used.

If you join our newsletter, we keep your first name, email, and if you choose, your mobile number and the treatments you are curious about, until you unsubscribe. Every email has an unsubscribe link, and you can reply STOP to any text.

Links to Jane, Facebook, Instagram and TikTok take you to those sites, which have their own privacy policies.

Email and social media messages are not fully secure. Please do not send detailed health information that way. Call or text us instead, or share it during your appointment.

How we protect it

• Electronic records are kept in secure, password protected systems.
• Any paper records are stored securely and kept locked.
• Access is limited to the people who need it to care for you.
• When records are no longer needed, they are destroyed securely.

If your information is ever lost, stolen or seen by someone who should not have seen it, we will let you know as required by law.

How long we keep it

We keep your health record for as long as required by the professional and legal rules for health records in Ontario. After that, it is securely destroyed.

Your choices and rights

You have the right to:

• Ask for a copy of your health record.
• Ask us to correct anything that is wrong or incomplete.
• Withdraw or limit your consent. We will explain how this may affect your care.
• Ask how your information has been used or shared.

Please send your request in writing to info@cliniquelarrivee.com. We will reply within 30 days. A small fee may apply for copies, and we will tell you about it first.
